Yoga Waiver

I understand that yoga includes physical movements and postures, as well as breathwork and meditation. As is the case with any activity, the risk of injury or trauma, even serious or disabling, is always present and cannot be entirely eliminated.

I assume full responsibility for any and all damages that may be incurred through my (or my minor’s) participation.

Yoga and yoga therapy are not substitutes for medical attention, examination, diagnosis, or treatment. Yoga therapists and yoga teachers are not qualified to diagnose, treat, or prevent any disease. Yoga is not recommended and may be unsafe under certain medical conditions.

By signing, I affirm that a licensed physician has verified my condition to participate in such a program. IN ADDITION, I WILL MAKE THE INSTRUCTOR AWARE OF ANY MEDICAL CONDITIONS, PHYSICAL LIMITATIONS, HEALTH RESTRICTIONS, QUESTIONS, OR CONCERNS BEFORE ANY CLASS, SESSION, PROGRAM, OR WORKSHOP.

If I am pregnant, become pregnant, or I am post-natal or post-surgical, my signature verifies that I have my physician’s approval to participate. I also affirm that I am solely responsible for deciding whether to practice yoga and that participation is at my own risk.

I hereby agree to irrevocably release and waive any claims that I have now, or may have hereafter, against J Hoffman Therapy, LLC dba Instar Healing, its instructors, and the owners of premises where classes, sessions, workshops, or programs are held.

I acknowledge that participation could result in COVID-19 infection. Accordingly, in addition to all waivers and limits on liability already agreed to by the parties and because of the COVID-19 Pandemic, the undersigned, HEREBY WAIVES AND RELEASES, indemnifies, holds harmless and forever discharges J Hoffman Therapy, LLC dba Instar Healing, its instructors, and owners of premises where classes, sessions, workshops, or programs are held, of and from any and all claims, demands, debts, prosecutions, expenses, causes of action, lawsuits, damages and liabilities, of every kind and nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in any way related to participation in any of the events or activities conducted by, on the premises of, or for the benefit of, J Hoffman Therapy, LLC dba Instar Healing, provided that this waiver of liability does not apply to any acts of gross negligence, or intentional, willful or wanton misconduct; further, it is acknowledged that operation during the pandemic does not fall into these categories.

I hereby fully and forever release and hold harmless J Hoffman Therapy, LLC dba Instar Healing, and/or its instructors against any and all liability to me, my heirs, executors, personal representatives, administrators and/or assigns, for any and all claims, demands, causes of action, losses and damages of any kind whatsoever on account of injury including loss, injury, death or damage to my person and/or my property or to any other person and/or their property, cause or alleged to be caused by any action or inaction of J Hoffman Therapy, LLC dba Instar Healing, and its instructors.

I hereby waive the right to sue J Hoffman Therapy, LLC dba Instar Healing, and/or its instructors for any injuries or damages I may incur, whether known or unknown, resulting from my participation in any activity.

I understand and agree that this document is binding on myself, my heirs, personal representatives, executors, administrators, and assigns.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
